PHP Community Impact Partnership with UK Community Foundations

PHP are delighted to be working with UK Community Foundations to offer grants to charities and community groups that are focused on social prescribing and community wellbeing that serve our properties.

We have launched the project in two pilot regions in Scotland and Lincolnshire and are working with our GP practices, their social prescribing coordinators and link workers to promote the fund to the organisations delivering social prescribing services to their patient lists and local communities.
